FOMO token pages live at fomo.family/coin with the token address, FOMO's chain id and an optional referral code. The chain ids are 1399811149 for Solana, 8453 for Base, 56 for BNB Chain, 143 for Monad, 4663 for Robinhood Chain, 5042 for Arc and 1 for Ethereum.

Not necessarily. EVM addresses can repeat across chains, so the generator checks DexScreener, starts on the chain where the token trades most, and lists every chain it was found on so you can switch.
